It all begins at the review landing page. This page should have a large button that initiates the review capture process. Also from this starting page the patient should be able to read previous reviews left by other patients. Once the leave a review button is pressed, the patient proceeds through subsequent screens and begins answering questions to leave their review.
The review capture process consists of one question per screen. The patient selects one doctor from a list of doctors, then can optionally pick a procedure or treatment to assign to review, then selects a star rating from 1 to 5 stars, then types free text comments, then enters their email address, then enters their name , answers a yes/no question then agrees to a consent statement and is done.
We have struggled with how to best communicate the need to select 1 to 5 stars, so feel free to try lots of variations here (i.e. possibly always showing 5 stars, but including outlines/silhouettes for empty stars. So a 3-star choice would show three filled-in stars and two outlines). This is an important component to the process.
We are looking for a design for:
1. The landing page with a leave a review button and the ability to read previous reviews from other patients
2. The 1-5 Star selection screen
3. The free text email capture screen with a button/link with the answer to the "Why do you need my email address?" question
4. A single selection multiple choice screen, with a color indicating a selected button and a skip button
