There are three screens we are looking to have designed:
The "Home" page, where the user will first "land" when they visit the site. It should have a big, clear notification / button of any pending new requests (clicking it will take them to the next screen). It should have another big, clear button offering to let them provide feedback. It should have two additional buttons that allow them to view all sessions, as well as "the loop". Per the mockup, it should also contain the QI Path logo, as well as the user's information, and some "tools" for home, settings, and help.
The second page is the "Session" page, where it will display the session description. (http://vround.acgdemo.com/request.php?id=1). This page should provide a summary of the session details, with a small button / link to view full details, as well as a big clear button to go to the next page to actually complete the questions. In addition, it should have two buttons for "Saving for Later" and "Declining" to take the survey.
The third, page is the actual series of questions (http://vround.acgdemo.com/perceptions.php). It should list the question(s) that the administrator has decided to pose, and display them in a manner that makes it obvious that they are clickable. Clicking into one will take the person to the "Rating" for that item. Note that after someone rates an item and goes back to this third page, there should be a graphical representation of their rating.
The fourth page (and final page) is the page where the user is presented with the question, description, and a series of rating buttons to select from (http://vround.acgdemo.com/rate.php?processid=1). Note that we need to have a couple of "sets" of icons: One that allows 4 options, 5 options, and 7 options (think rating on a scale of 1-4, scale of 1-5, and scale of 1-7). We need the icons to match these different ratings, with the 3 different scales (the admin would choose whether the scale is 4, 5, or 7 choices).
