I know, a little crazy to read the title, but this is something hospitals already do. I just think we could do it better! Basically looking for a one-page flyer that lists 9 different options for care as well as the advantages and disadvantages of each. Ideally this is all on one page, but it's ok to do Choice 1, 2, and 3 on the front and Choice 4-9 on the back. I think it's important the form is colorful and bright and simple (I've attached some examples, but I'm totally open to other ideas that stray from those). I'm not opposed to pictures, but I don't really think they're necessary. If you include them, they should be tasteful and unobtrusive; probably no photographs. Thanks, Everyone!
